Workshops for Community members
TC SMP offers FREE delivery of evidence-based Stanford Chronic & Diabetes Self-Management workshops for community members in English and French. TC SMP has an active pool of 54 trained facilitators who are eager to bring their skills and passion to help empower community members with chronic conditions.
These workshops take place in settings such as faith-based organizations, residence buildings, libraries, community based organizations, health care organizations, places of work, etc.

Workshops for Health care providers
TC SMP offers FREE delivery of Choices and Changes: Clinician Influence and Patient Action, a course designed by the Institute for Healthcare Communication in English.
Choices and Changes (C&C) is directed to the needs of clinicians working within the rigors of contemporary health care practice. This workshop is meant to help facilitate this interaction and provide skills and techniques to providers to better support self-management and behaviour in their patients.
C&C workshops take place in settings such as acute care, specialty care, long-term care facilities, rehabilitation teams, family health teams, physician offices, etc. Participants benefit immensely from this free training. If you are interested in hosting a training in your health care facility, please return a completed application form to TC SMP.
